There are classes that you can take where you actually pour the porcelain into molds to make the face, body, arms and legs.  Then you gently sand the entire surface of the doll, attach all parts and they bake it in a kiln.  When everything is dry, you paint and put on a finishing glaze.  If this sounds a little too complicated, there are many sites that sell kits to make these dolls.  http://www.dollhaven.net/catalog.
